Puppy Contract

The Buyer shall keep the dog as a household pet.

I understand

The Buyer shall provide the dog with an adequate food, fresh water, shelter, affection, and medical care upon sickness, disease, or injury. Maintenance should include an annual health exam and routine vaccinations as recommended by a board-certified veterinarian. Humane care and maintenance in accordance with all current and future state, county, and municipal laws and ordinances where the Buyer resides. A fully fenced yard or other humane means of exercise. The dog shall not be kept chained or constantly outdoors. A collar and identification tag and/or microchip. Never permit the dog to roam unattended.

I understand the dog shall not be sold, transferred, or given to any individual or to any retail or wholesale establishment for the purpose of sale or resale to the public or to any research institution where medical experimentation or other practices take place. *

I understand that my puppy is designated as pet quality and that I am required to spay/neuter my puppy in accordance with my veterinarian's best practice. *

I understand that I am required to have a well pet check up within 48 hours of receiving my puppy and that if I don't the health agreement will be terminated. *

I understand that I am required to notify Doodles of Madison County before transfering ownership of my puppy to any other person or entity. *

I understand that I can contact Doodles of Madison County if I ever have questions about my puppy, free of charge. *

I understand

The Buyer understands for the life of the dog, the Buyer may return the dog to the Breeder at any time and for any reason. If the Buyer does return the dog the Buyer agrees to sign the necessary papers to return the Breeder as sole owner of the dog. The Buyer also understands returning the dog to the Breeder for any other reason that is outlined in the health agreement the Buyer has no expectation of a refund and the Buyer incurs the cost of shipping/transporting the dog to the Breeder. *

Upon completion of payment, at no earlier than eight weeks old, the Buyer will receive from the Breeder, a Swissy Mountain Doodle, ACHC Litter F20-15243X (X=A through G depending on which puppy). This will be a puppy from Emily (Emmy), AKC WS61270801 and Famously Auston Theda (Charlie), AKC PR21084005. Please designate which puppy(ies) you are contemplating in the space below. *
